If you have lost your Medicare card, don’t panic. A lost Medicare card can be replaced with a few easy steps. Your Medicare card is your proof of insurance, and it may be required when you need health care treatment or prescription coverage. You’ll want to get a replacement card as soon as possible. If you haven’t lost your Medicare card, but your current card is illegible, stolen, or in some way destroyed you can get a replacement card.
The easiest way to get a replacement card is by visiting your “My Social Security” account. You don’t have to collect social security benefits to have a social security account. If you don’t have an online social security account, you can create one. Just go to the Sign in or Create an Account option on the page.
Once logged in to your account, select “Replacement Documents.” Then choose “mail my replacement Medicare card.” Your card will arrive in the mail in about 30 days from the day of the request. Please note your replacement card will be mailed to the address on file with the Social Security office.
If you are creating a My Social Security account for the first time to get a replacement Medicare card, you must provide some information about yourself and meet a few requirements.
Also, keep in mind when setting up an account that you can only create an account for yourself. You are not permitted to create an account for someone else to assist them with a lost Medicare card.
For example, you are not permitted to create an account for someone with whom you have a business relationship, someone for whom you are an authorized representative or an individual for whom you are an authorized payee.
If you don’t have an online account and do not wish to create one, you can still get a replacement Medicare card by contacting the U.S Department of Health and Human Services at 800-772-1213. The office is open Monday through Friday from 7 a.m. -7 p.m.
Your third option is to contact or visit your local social security office.
